In consideration of my participation (and/or participation of a MINOR as the Legal Guardian) in Encore Sports Company sanctioned events, I agree to the following:
1. READINESS TO COMPETE: I will only participate in those Encore Sports Company events and competitions for which I believe I am physically and psychologically prepared to compete. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation especially any head or spinal trauma that could be concussion related, I will remove myself from participation and bring such to the attention of the nearest official immediately and if necessary seek medical consultation for clearance to participate. As a condition of my readiness to compete I have not experienced symptoms that of fever, fatigue, difficulty in breathing, or dry cough or exhibiting any other symptoms relating to COVID-19 or any communicable disease within the last 7 days.I have not, nor any member(s) of my household, traveled by sea or by air, internationally within the past 14 days.I did not, nor any member of my household, visit any any area within the United States that was reported to be highly affected by COVID-19, in the last 14 days .I have not been, nor any member(s) of my household, diagnosed to be infected of COVID-19 virus within the last 14 days.
2. MEDICAL CONSENT: I hereby give my consent to Encore Sports Company and the host organization of my Encore Sports Company sanctioned event to provide, through a medical staff of its choice, customary medical/athletic training attention, transportation and emergency medical services as warranted in the course of my participation in Encore Sports Company and sanctioned events. I agree to be financially responsible for any costs incurred as a result of such treatment. I am aware that Encore Sports Company does not provide health insurance for me and that I should carry my own health insurance
3. ASSUMPTION OF RISK WAIVER, RELEASE OF LIABILITY AND COVENANT NOT TO SUE. The risk of injury from the activities involved in this program is significant, including the potential for physical injury or illness such as, but not limited to influenza, MRSA, COVID-19 that may lead to pain, suffering, disfigurement, temporary or permanent disability, paralysis, death, or economic loss, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist. I am fully and personally responsible for my own safety and actions while and during my participation and I recognize that I may be in any case be at risk of contracting COVID-19.I am aware there are risks to me of exposure to directly or indirectly arising out of, contributed to, by, or resulting from an outbreak of any and all communicable disease, including but not limited to, the virus, severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), which is responsible for Coronavirus Disease (COVID-19) and/or any mutation or variation thereof. Specifically, I affirm that I know, understand, and appreciate the inherent risks of the activities organized, administered, or sponsored by or associated with Encore Sports Company and sanctioned events (each, an "Activity" and collectively,"Activities"). I affirm that I am voluntarily participating in the Activity and further acknowledge that I know, understand, and appreciate the inherent risks of any Activity or Activities. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, release, waive and covenant not to sue Encore Sports Company, its officers, directors, members and agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event ("RELEASEES"), with respect to any and all injury, disability, death, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.I fully accept and assume all such risks, whether forseeable or not, even if arising from the negligence of the releasees or others and all responsibility for any losses, costs, expenses, liabilities and damages that may be incurred as a result of or in connection in any way with my participation including travel to and from the Activity (including air travel) or any events incidental to any Activity I agree to these terms freely and without any inducement or assurance of any nature and intend it to be a complete and unconditional release of all claims and liability to the greatest extent allowed by law and agree that if any term or condition herein is held to be invalid or unenforceable for any reasons that all other terms and conditions hereof shall continue in full force and effect. I agree to hold Encore Sports Company harmless from any and all claims, loss or damage to my personal property, liabilities and costs, including attorney's fees, as a result of my participation in any Activity, including travel to and from an Activity (including air travel) or any events incidental to this Activity. If Encore Sports Company incurs any of these types of expenses, I agree to reimburse Encore Sports Company.
Encore Sports Company Code of Conduct
The Encore Sports Company Code of Conduct is strict and serious in our sport. All people associated with the game are always expected to act with sportsmanship, dignity and respect for others. This includes players and referees.

Poor sportsmanship will carry, at a minimum, the following consequences:
I understand that events, games and activities organized by Encore Sports Company rely on electronic communications via email, SMS text and/or mobile app notifications to convey program changes, policy updates and attendance. If any activity reaches maximum attendance advertised on any electronic tracking application used by Encore Sports Company the participants will honor the attendee limit and not attempt to participate in said activity over and above the attendee limit. Participants will honor any "no show" policies in effect and accept any penalty assessed if the policy is violated.
All participants are expected to adhere to common sense decency and mutual respect of fellow participants during organized activities AND when using electronic communications used by Encore Sports Company. Any violation of protective classes or use of vulgar language directed to another participant will not be tolerated and is subject to permanent ban from any activities organized by Encore Sports Company
I have read the Encore Sports Company Code of Conduct and agree to abide by it as part of my participation. I understand that there is no refund or recourse for dismissal from our program as a consequence of breaking this Code of Conduct.
Encore Sports Company Photo/Video Consent
I hereby grant permission to Encore Sports Company to use photographs and/or video of me taken during participation in any competition or event organized by Encore Sports Company for use in publications, news releases, online, and in other communications.
FOR PARTICIPANTS OF MINORITY AGE (UNDER AGE 18 AT THE TIME OF REGISTRATION)
This is to certify that I, as parent/guardian, with legal responsibility for this participant, have read and explained the provisions in this waiver/release to my child/ward including the risks of presence and participation and his/her personal responsibilities for adhering to the rules and regulations for protection against communicable diseases. Furthermore, my child/ward understands and accepts these risks and responsibilities. I for myself, my spouse, and child/ward do consent and agree to his/her release provided above for all the Releasees and myself, my spouse, and child/ward do release and agree to indemnify and hold harmless the Releasees for any and all liabilities incident to my minor child's/ward's presence or participation in these activities as provided above, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent provided by law.
